Welcome to C-CCA. We hope that the material contained on this website will provide important information and answer some questions prior to your first visit. If you wish, you can complete most of the necessary paperwork in advance of our first meeting by doing the following:

Please read the Confidentiality Information form or the HIPPA form. We will briefly discuss them with you when we meet. After we answer any questions you might have, you can then sign an informed consent and the paperwork will be finished.
If you prefer, we can complete the paperwork at your first session instead. We will be happy to answer any questions you might have regarding this information when we meet. Below is a downloadable list of all of the documents.
